I am an out-of-network provider. Please contact your insurance company for eligibility of reimbursement. If you choose to use insurance, I will provide the required information on a superbill that you will need to submit to your insurance company.
I have opted out of Medicare, which means I see Medicare clients on a private-pay basis and our sessions will not be reimbursed.
As stated in the "No Surprises Act," you are entitled to clear and accurate information about your therapy charges. Your annual investment in therapy will vary based on the services you utilize. This includes how often you participate in therapy and if you have insurance benefits for therapy services and choose to use the benefits. Below are the fees for therapy services.
The full session fee is due at the time of service. Insurance reimbursement is handled by the client.
A credit card is kept on file for ease of payment and timely receipts.
Cancellation Policy
If you are unable to keep your scheduled appointment, please cancel within 24 hours to avoid a full session fee.
Forms
Once the first session is booked, required forms will be sent via a secure document signing service to complete, sign, and submit before your first session.
